I've read elsewhere that Fox staff will sometimes demand proof your credit card covers the insurance you decline. Does anyone know what sort of proof they accept?

They didn't go that far with me, but they do challenge you a bit. I said I had the card, and my regular insurance to cover what the card would not and she asked the carrier name and whether they covered cars in the US.

Wow, I've never had that problem with Fox, and they are my go-to rental.

    My counter experience is:
"Do you want insurance? [The world will end if you get into an accident]”
    "No, thanks."
"Are you sure? It's not that expensive..."
"I'm sure, thanks. And I'll take care of the gas myself."
Type, type, type, off I go.

I wondered this as well. We rent strictly for the days we need and directly at the hotel property. It’s much cheaper that way, and easier, imo.

The problem with FOX and Payless and those off brands is that might be the case most of the time, but often you'll get the hard seller that wont let you off the hook that easy. Then they may hit you with odd requirements like @GustavsBar mentioned above. Its just not consistent from location to location. Again, for the little amount of money you are saving, i dont see why you need this hassle on your vacation. Personally, if i can't go straight to the car then im not interested.
