MGM W2-G statement

    I recently lost my W2-G form from my last trip. How could I go about getting a new one? I already spoke to 5 different departments within Aria and Mlife. They all could not help me. Most did not even know what a W2-G form was and kept referring me to the online site for my Win / Loss Statement. I know I need to fax a request form but no one seems to have the form.

    The win/loss statements on the MLife site have this at the bottom:

    The phone number is probably useless but you can try the email address.
