Tried it on May 11th with no luck, $20 declined and returned. However my request for a room on the higher floors was accepted.

It worked for me in June 2015 on a Fri, Sat, Sun stay. I was already getting those 3 nights free so I didn't think I would have much of a chance. They didn't have a suite but they gave me a much bigger room with a mini fridge and a view of freemont street. It was well worth the $20.

September 2016. Arrived Tuesday night about 9PM. While I didn't try the triditional sandwich. I laid my wallet on the desk with a $20 pulled 1/2 way out. Asked if there were any complimentary suite upgrades available. After about 5 minutes of computer searching was told only one suite was available and it would be $50/night. I declined and requested a quiet room closer to Ogden and was assigned a room on the 14th floor facing Binions. No $20 needed.

Has anyone been upgraded with a tip, say a $20 one, at the Fremont?

Checked into the Fremont on 11/6, and the $20 sandwich scored a mini-suite on the 14th floor. Suite was great, and we really enjoyed the Fremont.
