Other than down town or the Palms, Rio, Orleans, what other off strip casinos would you say is worth checking out for a day?

Hard to answer w/o knowing what you are looking for. If you want full pay VP, any of the Stations will work. Red Rock and Green Valley Ranch being the two nicest. M has great architecture and buffet. Southpoint has good gambling in a somewhat plain casino. Silverton has the big fish tank and Bass Pro Shop. Sam's has the Atrium and laid back vibe. Two or three tiny dark places in Downtown Henderson, (completely different than anything else you'll run into). If you're looking for table games, any one of the bigger casinos will work just fine. We like the three casinos nw along rancho drive, but there's really nothing special about them (except I got my first royal at Fiesta). Your Avatar says you've been to LV 18 times already. Rent a car and go check them all out, as well as some other sightseeing!!

Aliante and M are both worth checking out if you feel like driving or if you are in those areas for any reason.

If I had to pick one, I'd say Red Rock.

I agree with the above poster, Red Rock is on my to-do list. Is SLS considered off strip? Been there a few times and had fun, good music, friendly staff.

Ladies getaway, somewhere on the strip with a pool :)

For off-strip properties, I am fond of:

    Silverton
    M Resort
    Red Rock

On my last trip I spent an afternoon at Gold Coast and was pleasantly surprised.
I found 9/6 JOB and more importantly for me, a double deck 3/2 pitch BJ game at $5!
The atmosphere is the opposite of the strip vibe. Quiet, low key and no crowds. It was a nice break.

Gold Coast is a nice casino for low budget gambling. If you like Gold Coast, you'll love South Point except for the 9/6.

Red Rock has a ton of restaurants, it's connected to Downtown Summerlin for more restaurants and shopping, and if you have a car, it's a pretty cool drive out to Red Rock.

For low rolling we loved the Cannery and Cannery East. It's been a while, but they used to have a value priced buffet that we earned a comp with about an hour of play.

Off-strip properties are fun to visit, even if only for the "collect a new player's club card". I actually have one from Eureka Casino, home of the "Fat Choy" Chinese restaurant.

However, IMHO, gambling is gambling. The light show at Sam's Town's Mystic Park is nice to see once . . .once being the operative word. If you're going to Player's Club collect, unless there's two of you and one of you doesn't mind staying in line, pick the off hours. I didn't get a Player's Club card at Cannery Row last time because the line was just too long.

I go to Las Vegas so often that I'm looking for non-gambling things to do. . . .

OP mentioned "Other than Downtown" so that's why no one is bringing up Fremont.

Ellis Island if you like BBQ or for cheap good breakfast.

I would hate to narrow it to just one, but if I had to, I would probably suggest Red Rock. But you could theoretically cram my other four favorites in a day without too much travel.

My other favorites in no particular order are GVR, South Point, M Resort, and Silverton

If you mostly like slots, I probably pick Red Rock or GVR first. Very good buffets too. Note: Both of their casinos are very similar layout (both Stations properties), so visiting both might be overkill.
If you like "gimmick" VP and want good and often "full pay" at quarters, I would pick South Point. I also like their deli and sportsbook hot dogs.
Like craft beer and VP, M Resort and their 32 degree bar (so if I'm drinking this is my clear favorite...lol)

Want to see a mermaid in a tank, a bass pro and a casino with a more rustic look, there's Silverton. lol That's probably the bottom of my top 5.

Basically looking for a different experience then on the strip and downtown.
Since would probably spend a few hours, having a good choice of food from which to select would be nice too. If they have any 24 hour places to eat would be a plus.
Decent table game and nice selection of slots is a plus.

Thanks for all the replies. Will spend time googling each for more info and locations as well as which may have shuttles from the strip.

I am pretty sure that Red Rock has a shuttle that goes to Fashion Show Mall and I think M has one that goes to Tropicana (or near Tropicana). I'm sure their websites have the shuttle schedule.

The m shuttle picks up at Fashion show as well. I think it's technically for hotel guests only but I've never had an issue getting on.
