Need some tax help - not casino related

    I’ve been doing my own taxes since my very first job and I’ve never had this before. I need some help and thought you would be a good resource.

    I started a new job Dec 2014. I received 1 last paycheck for a couple of days pay from the previous employer the first week in Jan 2015 for around $743 net. What is strange is that on that paystub it shows only $1.50 in federal taxes. My exemptions were 3. I never noticed it until when I got my W2.

    That’s the first strange thing.

    The 2nd is that I received a paystub of sorts this past December that said “Taxable Fringe Benefit” in the amount of $885 – but no net pay or anything. I had no idea what that was for at first. It was showing as gross pay but no taxes and no net pay.

    Then I get the W2 and I’m now supposed to report that $885 as income and pay tax on it.

    I asked my employer what that was and they said “unsubstantiated FSA claim” – which cannot be right because I sent in all of my required paperwork for FSA before the end of my employment there. Unless they lost it. I remember exactly what the $885 was for, it was my daughter's balance on her braces which I did at the very beginning of 2014! And I sent the required ppwk to them right away.

    They gave me a number to some company handling those and when I called the company, they can’t even find me in the system.

    SO –

    #1, They cannot provide me proof that I didn’t send in ppwk for my FSA (and I know I did)

    #2, I looked up online and it said somewhere that FSA cannot be reported as a taxable fringe benefit. Which is how it is listed on my W2.

    #3, the FSA was something I had used in 2014, not 2015. I didn't work for this employer past Dec 23 2014.

    #4, That $1.50 in fed taxes on my last check seems to be wrong doesn’t it?

    I’m so confused. I’m not going to report this $885 as income and pay taxes on it if it is not correct. I keep badgering them for more information and they keep putting me off to other people and nobody can help. I’d like to file my taxes as soon as one more paper comes in that I need to file. I don’t want to have to wait on them.

    I cannot log into my old FSA account to show where I uploaded the FSA receipts with the date noted .
